Here's what you need to do when you're unable to add an attachment to the _Raise an incident_ form from the requester portal:
1\. Go to the Application tab on your Browser
2\. Navigate to Storage -> Cookies -> select the portal ( acme.freshservice.com ) from the list of cookies.
3\. Right-click and select the clear option.
4\. Reload the page and attach the file
